Diastolic Heart Failure — Abnormalities in Active Relaxation and Passive Stiffness of the Left Ventricle

ABSTRACT
Background Patients with signs and symptoms of heart failure and a normal left ventricular ejection fraction are said to have diastolic heart failure. It has traditionally been thought that the pathophysiological cause of heart failure in these patients is an abnormality in the diastolic properties of the left ventricle; however, this hypothesis remains largely unproven.
Methods We prospectively identified 47 patients who met the diagnostic criteria for definite diastolic heart failure; all the patients had signs and symptoms of heart failure, a normal ejection fraction, and an increased left ventricular end-diastolic pressure. Ten patients who had no evidence of cardiovascular disease served as controls. Left ventricular diastolic function was assessed by means of cardiac catheterization and echocardiography.
Results The patients with diastolic heart failure had abnormal left ventricular relaxation and increased left ventricular chamber stiffness. The mean (±SD) time constant for the isovolumic-pressure decline ( ) was longer in the group with diastolic heart failure than in the control group (59±14 msec vs. 35±10 msec, P=0.01). The diastolic pressure–volume relation was shifted up and to the left in the patients with diastolic heart failure as compared with the controls. The corrected left ventricular passive-stiffness constant was significantly higher in the group with diastolic heart failure than in the control group (0.03±0.01 vs. 0.01±0.01, P<0.001).>50 percent) who had been scheduled for diagnostic cardiac catheterization were evaluated for participation in the study. All the patients met the Framingham criteria for congestive heart failure.20 Patients with pulmonary disease, renal disease, heart-valve disease, atrial fibrillation, or evidence of hypertrophic cardiomyopathy were excluded. Echocardiographic indexes of diastolic function were not used as inclusion criteria. Forty-seven patients met the inclusion criteria and underwent combined catheterization and echocardiographic studies. Data on the left ventricular end-diastolic pressure and left ventricular filling dynamics in these 47 patients have been reported previously.21 However, data on these patients' left ventricular volume and passive stiffness (the relation between diastolic pressure and volume) have not been previously published. All 47 patients met the criteria of Vasan and Levy for definite diastolic heart failure. There were 31 men and 16 women; their mean (±SD) age was 59±12 years. The 10 controls had a mean age of 58±16 years. They underwent cardiac catheterization for the evaluation of symptoms of chest pain. They had no history of cardiovascular disease, and all 10 had angiographically normal epicardial coronary arteries and normal left ventricular volume, ejection fraction, and wall motion.
Cardiac Catheterization
Cardiac catheterization was performed with the use of standard techniques. A high-fidelity micromanometer catheter was placed in the left ventricle, Doppler echocardiographic recordings were obtained, and left ventricular pressures and the time constant of the isovolumic-pressure decline ( ) were measured. The left ventricular minimal diastolic pressure was defined as the lowest pressure after the opening of the mitral valve, the left ventricular diastolic pre–A-wave pressure was defined as the diastolic pressure before atrial contraction, and the left ventricular end-diastolic pressure was defined as the pressure after atrial contraction, just before the rise in left ventricular systolic pressure. Pressure data were digitized at intervals of 5 msec, and was calculated by the method of Weiss et al.22 Analysis of the pressure data was performed in a core laboratory.21
Echocardiography
The dimensions and wall thickness of the left ventricle were measured according to the recommendations of the American Society of Echocardiography. Calculations of left ventricular volume and mass were made with standard published methods.23 Analysis of the echocardiographic data was performed in a core laboratory.21 Total stroke volume was calculated on the basis of the echocardiographic measurements. The increment in left ventricular diastolic volume that occurred between the opening of the mitral valve and the point of minimal diastolic pressure was calculated first by determining, with the use of Doppler echocardiographic techniques, the fraction of the total left ventricular inflow velocity integral that occurred during this period. Next, the total stroke volume was multiplied by this fraction and added to the end-systolic volume. The result was the volume at the time of minimal diastolic pressure. A similar technique was used to calculate the volume at the point of pre–A-wave pressure.
Calculation of Passive Diastolic Stiffness
Diastolic stiffness was assessed with the use of three left ventricular diastolic pressure–volume coordinates: end-diastolic pressure and volume, pre–A diastolic pressure and volume, and pressure and volume at the time of minimal diastolic pressure. The diastolic pressure–volume relation can be described by an exponential equation, P=Ae V, where P is the left ventricular diastolic pressure, V is the left ventricular diastolic volume, and A and are curve-fitting constants used to quantify passive stiffness.4 We reasoned that the abnormally slow rate of ventricular relaxation in our patients with diastolic heart failure would preclude full relaxation of the myocardium in early diastole. Thus, incomplete relaxation at the point of left ventricular minimal diastolic pressure might cause the latter value to be higher than it would be if it reflected the purely passive stiffness of the ventricle. Therefore, a corrected value of left ventricular minimal diastolic pressure was obtained by subtracting the contribution to pressure of slow (or incomplete) relaxation from the measured pressure value. This corrected pressure was used to calculate a corrected passive-stiffness constant.
The method used to make this correction, illustrated in Figure 1, is based on standard engineering concepts and published experimental data.24,25,26,27,28 In brief, the time course of the decline in left ventricular pressure was plotted from the point of aortic-valve closure to the time that the left ventricular pressure would approach zero if relaxation proceeded in the absence of filling. This approach was based on the concept that relaxation is essentially complete after a time interval equal to 3.5 .24,25,26,27,28 Then the time from the closure of the aortic valve to the point of left ventricular minimal diastolic pressure was measured, and the pressure at this time was determined from the plot of the natural log of pressure versus time. This contribution of slowed relaxation to pressure represents the extent to which the left ventricular minimal diastolic pressure exceeds the purely passive pressure. By subtracting the pressure contribution of slowed relaxation from the measured value of the left ventricular minimal diastolic pressure, we obtained a corrected left ventricular minimal diastolic pressure. This corrected pressure was used to calculate the corrected passive-stiffness constant.

Statistical Analysis
The 47 patients with definite diastolic heart failure had participated in a previous clinical trial21; the 10 controls had not participated in that trial. The sample size for the control group was based on the hypothesis that the mean value for the corrected stiffness constant in the group with diastolic heart failure would be two times as high as that of the control group. Therefore, using a sample size of 47 patients with diastolic heart failure and 10 controls would yield 80 percent power to detect a twofold difference in the corrected stiffness constant with the use of a conservative standard deviation of 0.01 and a two-sided alpha level of 0.05. Differences between the group with diastolic heart failure and the control group were examined with the use of an unpaired t-test. A P value of less than 0.05 was considered to indicate statistical significance. All reported P values are two-sided. Data were collected at each of the seven study sites and analyzed in the two core laboratories. Additional data analysis was performed and the manuscript was written by the authors.
Results
Left Ventricular Active Relaxation
In the patients with diastolic heart failure, was abnormal, the left ventricular early minimal diastolic pressure was increased, and there was a positive correlation between and the minimal diastolic pressure (R2=0.77). In these patients, the left ventricular minimal diastolic pressure occurred a mean of 138±5 msec after closure of the aortic valve, which occurred before 3.5 (i.e., before 218±5 msec). Thus, relaxation was incomplete at the time of left ventricular minimal diastolic pressure in all the patients with diastolic heart failure; incomplete relaxation accounted for 7±1 mm Hg of the measured value of this pressure. By contrast, relaxation was complete at the time of left ventricular minimal diastolic pressure in all the controls.
Left Ventricular Passive Stiffness
The end-diastolic pressure was higher and the end-diastolic volume was lower in the group of patients with diastolic heart failure than in the control group (Table 1). These data alone suggest the presence of increased chamber stiffness in the patients with diastolic heart failure. As shown in Figure 2A, the entire diastolic pressure–volume relation was displaced up and to the left in the patients with diastolic heart failure as compared with the controls. The left ventricular chamber-stiffness constant and curve-fitting constant, when calculated on the basis of measured values, were higher in the patients with diastolic heart failure than in the controls. When the corrected values for left ventricular minimal diastolic pressure were used to calculate the chamber stiffness (Figure 2B), the difference in passive stiffness was even more pronounced. As shown in Figure 3, the chamber-stiffness constant was higher in the patients with diastolic heart failure than in the controls.

Discussion
This study provides evidence that patients with heart failure and a normal ejection fraction have abnormalities in the diastolic properties of the left ventricle that are sufficient to explain the patients' hemodynamic abnormalities and occurrence of heart failure. The patients in this study who had clinical evidence of heart failure with a normal ejection fraction and who met the criteria of Vasan and Levy for definite diastolic heart failure had abnormal active relaxation and increased passive stiffness.20 These results support the hypothesis that the presence of increased diastolic pressures and signs and symptoms of heart failure in patients with a normal ejection fraction are characterized, if not caused, by abnormalities in the diastolic properties of the left ventricle. Therefore, the term "diastolic heart failure" can be appropriately used to describe the abnormalities in such patients.
Cardiogenic pulmonary edema in patients with diastolic heart failure is often the result of sodium retention and expansion of the central blood volume.7 Neurohormonally mediated increases in venous tone and systemic arterial pressure may contribute to a shifting of the blood to the central circulation and thereby cause a substantial increase in left ventricular diastolic pressure in such patients.29 Alterations in arterial stiffness may also contribute by exacerbating the underlying abnormalities in active relaxation and passive stiffness. However, none of these individual factors (sodium retention, neurohormonal activation, increased venous tone, or increased arterial stiffness) cause heart failure in patients with normal left ventricular structure and function. It is the increased left ventricular stiffness in patients with diastolic heart failure that makes them especially vulnerable to the development of pulmonary edema. Increased passive stiffness of the left ventricle dictates the association of very small changes in volume with large changes in left ventricular diastolic pressure.4,5 Indeed, significant changes in pressure may be seen even with little or no detectable change in ventricular volume.29,30 Thus, pulmonary edema in patients with diastolic heart failure is the direct consequence of increased passive chamber stiffness; the ventricle is unable to accept venous return adequately without high diastolic pressures. Such high filling pressures result in decreased lung compliance, which increases the work of breathing and contributes to dyspnea.
The mechanisms responsible for chronic exercise intolerance (particularly dyspnea and fatigue with effort) in patients with left ventricular diastolic dysfunction are less well understood. However, at least two mechanisms play a role, and both are dependent on abnormalities in diastolic function.31 First, patients with diastolic dysfunction (even those with little or no elevation in left ventricular filling pressure at rest) have a substantial increase in left ventricular diastolic pressures and pulmonary venous pressures during exercise and a significant limitation in exercise tolerance.30 This mechanism is similar to the mechanism that underlies dyspnea in patients with pulmonary edema.29 Second, the noncompliant stiff ventricle has a limited ability to use the Frank–Starling mechanism.30,31 As a result, patients with diastolic dysfunction have little or no increase in stroke volume during exercise, a finding that correlates positively with symptoms of exercise intolerance.30,31 Therefore, during exercise, the small, stiff ventricle in patients with diastolic dysfunction is unable to fill optimally, and despite an increased filling pressure, the cardiac output cannot increase. Thus, the exercise intolerance in patients with diastolic heart failure is the direct consequence of abnormal left ventricular diastolic function.
A number of methods have been used to evaluate left ventricular diastolic stiffness.24,32,33,34 Pak et al. showed that there is agreement between measurements of passive chamber stiffness obtained with "single-beat analysis" (i.e., multiple pressure–volume coordinates from a single beat) and those obtained with "multiple-beat analysis" (i.e., end-diastolic pressure–volume coordinates from multiple beats) in patients with hypertensive heart disease, the disease process that most often leads to diastolic heart failure.33 For this reason, and because the multiple-beat method is difficult to apply in large clinical studies, we used a single-beat method of analysis. However, the accuracy of the curve-fitting constants derived from the pressure and volume data is dependent on a number of factors, including the number of data points used in the calculation. In addition, the effects of slow or prolonged relaxation on early diastolic pressure can limit the accuracy of the single-beat method. In the current study, we avoided this problem by correcting the early diastolic pressures for delayed relaxation. In the patients with diastolic heart failure, such a correction yielded substantially higher passive-stiffness constants than those obtained with the traditional, uncorrected method.
Many, but not all, of the structural, functional, and demographic features of the patients in the current study are shared by the patients examined in recently reported epidemiologic community studies of patients with diastolic heart failure.7,8,9,10,11,12 For example, more than 75 percent of the patients in both the current and the previous studies had hypertension. Thirty-eight percent of the patients in the current study had left ventricular hypertrophy (defined as a left ventricular mass that exceeded 125 g per square meter of body-surface area). These findings are consonant with data from the studies by Chen et al.7 and Kitzman et al.,10 in which 43 percent and 35 percent of patients with diastolic heart failure, respectively, had left ventricular hypertrophy. Therefore, left ventricular hypertrophy is a common finding in patients with diastolic heart failure; its presence supports but is not required for a diagnosis of diastolic heart failure.20,21 Patients with diastolic heart failure generally have normal or even small left ventricular chamber volumes.10,13,35 Echocardiographic studies have shown that the left ventricular end-diastolic volume is 103±20 ml (where the normalized value indexed to body-surface area is 56±10 ml per square meter) in normal persons but 90±18 ml (where the normalized value indexed to body-surface area is 47±5 ml per square meter) in patients with diastolic heart failure.10,13,35,36,37,38,39,40 Data from the current study also indicate that left ventricular volumes are normal or decreased in patients with diastolic heart failure.
Despite these many similarities, there are some differences between the current study and previous studies.6,7,8,9,10,11,12 For example, the patients in the current study were younger than those in the previous studies, and a higher proportion of them were men. However, each of the patients examined in the current study fulfilled the criteria of Vasan and Levy for definite diastolic heart failure — a requirement the epidemiologic studies were not designed to meet. Therefore, we believe that the conclusions made in the current study are applicable to a wider population of patients, such as those described in the previous studies.6,7,8,9,10,11,12
Patients who meet the criteria for definite diastolic heart failure have abnormal active relaxation and increased passive stiffness. The predominant pathophysiological cause of heart failure in these patients is abnormal diastolic function. Therefore, it is appropriate to use the term "diastolic heart failure" to describe the abnormalities in these patients.

ABSTRAK

Kateterisasi jantung

Ekokardiografi

Dalam Panel A, penurunan tekanan ventrikel kiri adalah ilustrasi untuk tiga nilai dari waktu relaksasi konstan (). Misalnya, jika sama dengan 60 msec dan tekanan diastolik minimal awal (Pmin) terjadi 130 msec setelah penutupan katup aorta-(ditunjukan dengan panah vertikal), kontribusi relaksasi melambat menjadi tekanan (Psr) adalah 7 mm Hg. Analisis Statistik
47 pasien dengan gagal jantung diastolik pasti telah berpartisipasi dalam trial21 klinis sebelumnya; dari 10 kontrol tidak berpartisipasi dalam sidang itu. Ukuran sampel untuk kelompok kontrol berdasarkan hipotesis bahwa nilai mean untuk dikoreksi kekakuan konstan pada kelompok dengan gagal jantung diastolik akan dua kali lebih tinggi dari kelompok kontrol. Oleh karena itu, dengan menggunakan ukuran sampel dari 47 pasien dengan gagal jantung diastolik dan 10 kontrol akan menghasilkan 80 persen kekuatan untuk mendeteksi perbedaan dua kali lipat dalam kekakuan dikoreksi konstan dengan menggunakan deviasi standar konservatif sebesar 0,01 dan tingkat alpha dua sisi dari 0,05 . Perbedaan antara kelompok dengan gagal jantung diastolik dan kelompok kontrol diperiksa dengan menggunakan tes-t berpasangan. Hasil Waktu Relaksasi Aktif ventrikel
Dalam pasien dengan gagal jantung diastolik, tidak normal, tekanan diastolik ventrikel kiri meningkat minimal awal, dan ada korelasi positif antara dan tekanan diastolik minimal (R2 = 0,77). Pada pasien ini, tekanan diastolik ventrikel kiri terjadi minimal rata-rata 138 ± 5 msec setelah penutupan katup aorta, yang terjadi sebelum 3,5 (yaitu, sebelum 218 ± 5 msec). Jadi, relaksasi tidak lengkap pada saat tekanan diastolik ventrikel kiri minimal di semua pasien dengan gagal jantung diastolik; relaksasi lengkap merupakan 7 ± 1 mm Hg dari nilai yang diukur dari tekanan ini. Sebaliknya, relaksasi selesai pada saat tekanan diastolik ventrikel kiri minimal di semua kontrol.

Data-data ini sendiri menunjukkan adanya peningkatan kekakuan ruang pada pasien dengan gagal jantung diastolik. Diskusi
Studi ini memberikan bukti bahwa pasien dengan gagal jantung dan fraksi ejeksi normal memiliki kelainan pada properti diastolik ventrikel kiri yang cukup untuk menjelaskan kelainan hemodinamik pasien dan terjadinya gagal jantung. Pasien dalam penelitian ini yang memiliki bukti klinis gagal jantung dengan fraksi ejeksi normal dan yang memenuhi kriteria Vasan dan Levy untuk gagal jantung diastolik pasti telah relaksasi aktif normal dan meningkat stiffness.20 pasif Hasil ini mendukung hipotesis bahwa kehadiran tekanan diastolik meningkat dan tanda-tanda dan gejala gagal jantung pada pasien dengan fraksi ejeksi normal ditandai, jika tidak disebabkan oleh kelainan pada properti diastolik ventrikel kiri. Mekanisme yang bertanggung jawab untuk intoleransi latihan kronis (terutama dyspnea dan kelelahan dengan usaha) pada pasien dengan disfungsi diastolik ventrikel kiri kurang dipahami dengan baik. Sejumlah metode telah digunakan untuk mengevaluasi stiffness.24 diastolik ventrikel kiri, 32,33,34 Pak et al. menunjukkan bahwa ada kesepakatan antara pengukuran kekakuan ruang pasif diperoleh dengan "analisis satu-mengalahkan" (yaitu, tekanan beberapa volume-koordinat dari mengalahkan tunggal) dan yang diperoleh dengan "analisis multi-mengalahkan" (yaitu, tekanan diastolik akhir- volume koordinat dari beberapa beats) pada pasien dengan penyakit jantung hipertensi, proses penyakit yang paling sering mengarah ke jantung diastolik failure.33 Untuk alasan ini, dan karena beberapa metode-beat tidak sulit untuk diterapkan dalam studi klinis besar, kami menggunakan satu -mengalahkan metode analisis. Namun, akurasi dari konstanta kurva-fitting berasal dari tekanan dan data volume tergantung pada sejumlah faktor, termasuk jumlah titik data yang digunakan dalam perhitungan. Selain itu, efek relaksasi lambat atau lama pada tekanan diastolik dini dapat membatasi akurasi metode mengalahkan tunggal. Dalam studi saat ini, kami menghindari masalah ini dengan memperbaiki tekanan diastolik awal untuk relaksasi tertunda. Dalam pasien dengan gagal jantung diastolik, seperti koreksi menghasilkan pasif-konstanta kekakuan secara substansial lebih tinggi dari yang diperoleh dengan metode, tradisional tidak dikoreksi.
Banyak, tapi tidak semua, fitur yang struktural, fungsional, dan demografis dari pasien dalam penelitian ini dibagi oleh pasien diperiksa dalam studi epidemiologi baru-baru ini melaporkan komunitas pasien dengan hati, diastolik failure.7 8,9,10,11 , 12 Sebagai contoh, lebih dari 75 persen pasien baik di saat ini dan studi sebelumnya telah hipertensi. ABSTRAK

Background

There are two ways to open the abdominal cavum in elective surgery: analyze the transversal slices and vertical slices. Background

Choice of incision for open surgery of abdominal cavum can be based on the patient, operator, or the health care system criteria. From the points given patient to describe pain and restriction of the digestive tract is important. Considering previous research, probably most were not examined with attention to systematic or pesien. Attention of most surgeons, in addition to speed and to establish new optimal for field operations, is: time to open and the frequency of opening the abdomen, wound infection, post-operative pulmonary complications, and hernia insicional. For the economy, the parameters used such as the duration of surgery, LOS, and physical and mental activity in question.

Priority results from randomized studies and meta-analysis of field control has been suggested that pendekatam analyze the transversal is better than any other type of incision when considering the possibility of complications. But the practice of surgery with a midline incision is still a habit. One reason for this discrepancy may be due to lack of research and design, lack of internal validity. Method

Criteria requirements

Inclusion criteria: Age more than 18 years, life expectancy more than 12 years, procedures to be followed by the patient's plan:

1. Whipple Procedure / Pylorus-Preserve classic

3. Partial / total gastrektomy

4. Colon resection (left / right / analyze the transversal)

5. Ileocecal resection (primary or elective surgery, patients should be given informed consent, the patient had given informed consent)

Exclusive criteria:

1. Permanent therapy with opioid drugs for some reason during the 12 months before the operation (duration longer than 2 weeks)

Ethics, Informed Consent

The last procedure is approved by independent ethics committee Hedelberg University Medical School. Informed consent will be obtained from the patient either orally maqupun written before entering the study.

Security

Abdominal trauma, lung infection, and wound infection is a secondary end point, but also defined as adverse events (events are less well). Abdominal trauma and post operative infections will always dipertimbngakan as adverse events.

Limitation of adverse events include a few signs, symptoms, and syndrome, or diseases that appear or worsen during periods of observation in patients in clinical research and all that may damage the health of patients.

Statistical analysis

Alternative hypothesis of a primary endpoint is the remedy there is no relevant difference in pain postoperasi. Sample Size

Calculations of sample didasrkan at a = 0.025 (Bonferroni-Holm) and the desired power 0.80. Calculations were done separately in each primary variable using Query advisor version 4.0, Statistical Solutions Ltd.., Cork, Ireland. The maximum value of the sample size calculation should be taken as the sample size. The result was 86 patients per group (see below).

Painful experience

1. Design equivalence: Assume there is no difference between group means, SD 20 (VAS), and the equivalence margin d = 10 (VAS), 86 patients each group desired.

Program management

Monitoring

Follow-up

Latar belakang

Ada dua cara untuk membuka cavum abdomen pada operasi elektif: irisan tranversal dan irisan vertikal. Ketidaksesuaian dari dua rekomendasi yang ada, penelitian klinik dan praktek klinik dapat menerangkan kekurangan pada penerimaan atas hasil penelitian pada bagian deain penelitian dan analisisnya, juga pada rendahnya validitas internal, oleh karena itu generalisasi eksternalnya minimal. Obyek dari penelitian ini berdasar dari issue berasal dari issue persepsi pasien.

Metode

Adalah teknik random kontrol pada observasi intra operatif dan percobaan dengan pasien blinded dua group paralel equivalent. Penelitian dilaksanakan Department of general, Visceral, Tauma Surgery dan outpatient pada Medical School, Universitas Heidelberg. Total pasien adalah laki-laki dan perempuan berjumlah 172 orang, berumur dari 18 tahun yang direncanakan operasi abdomen elektif dan yang telah memenuhi syarat untuk dilakukan irisan tranversal atau vertikal. Untuk menunjukkan dua pendekatan tersebut atau lebih unggul salah satunya menurut persepsi pasien, dan endpoint pertama didefinisikan: pengalaman nyeri pasien (VAS 0-100) pada hari kedua setelah operasi dengan analgetik yang sama (piritramide [mg/h] ). Interval kepercayaan pada pendekatan ini akan digunakan untuk analisis. Level jaminan a = 0,05 dan powernya 0,8 menghasilkan masing-masing group berjumlah 86 orang. Latar belakang

Pilihan irisan untuk operasi membuka cavum abdominal dapat berdasarkan pasien, operator, atau kriteria system health care. Dari point yang diberikan pasien untuk menggambarkan nyeri dan pembatasan dari pencernaan merupakan hal yang penting. Mempertimbangkan penelitian terdahulu, mungkin sebagian besar tidak diperiksa dengan sistematis atau memperhatikan pesien. Masalah utamanya adalah penelitian yang kurang mempunyai kekuatan (jenis sampel terlalu kecil), untuk menestimasi perbedaan dengan selayaknya, standarisasi yang kurang dan atau kehilangan dari tehnik operasi penutupan cavum abdomen, tanpa binding pasien dan prasangka atau taksiran, penelitian yang tidak homogen pada meta analisis, dan kurang terstandarnya analgetik dan penganan post operatif. Salah satu meta analisis yang didapatkan menyimpulkan bahwa irisan tranversal merupakan lebih unggul dibandingkan irisan midline, pengurangan lebam pada kemunculan awal komplikasi (pembukaan abdomen dan penyakit pulmonal) dan komplikasi akhir (insicional hernia) yang berkaitan dengan anatomi dan fisiologi yang menguntungkan. Tetapi, kejadian komplikasi tidak hanya bergantung kepada tipe irisan, tetapi juga pada tehnik pembukaan. Penelitian termasuk telah dikembalikan kepada format awal dan diuji kembali secara terpisah sesuai point akhir yang disampaikan mereka (populasi).

Hanya satu Random Controlling Trial (RCT) terpusatkan pada frekuensi dari pembukaan abdomen dan tidak menemukan keuntungan dari pendekatan tranversal. Terjadinya komplikasi pulmonal awal terkaji pada RCT dan terdistribusi pada kedua group dengan sama. Pada tinjauan penelitian tidak memperlihatkan perbedaan yang signifikan pada terjadinya luka infeksi. Pada data asli Greenal dkk, jumlah hernia insicional mirip pada kedua pendekatan atau irisan. Pendek kata, data retrospektif dan prospektif studi dikumpulkan pada meta analisis, kemudian dapat dipastikan pada hasil yang tidak menyakinkan. Pada saat ini tidak ada evidence bahwa irisan tranversal adalah lebih unggul dibandingkan jenis insisis lainnya pada awal endpoint dan akhir-akhir komplikasi post operatif. Oleh karena itu, dapat dipastikan dengan memusatkan pada persepsi pasien. Untuk mendapatkan penelitian yang valid, pengamatan randomized control dan patient blinded study adalah kebutuhan standar untuk tehnik pembukaan, terapi nyeri, dan penanganan postoperatif. Penelitian ini bermaksud untuk menunjukkan tidak ada perbedaan yang signifikan dalam intensitas nyeri seperti yang dirasakan klien dimana mendapatkan piritramide dengan kebutuhan yang sama antara dua abdominal insicion, dengan asumsi mempunyai taksiran komplikasi yang mirip sama. Pokoknya awal dan akhir komplikasi dari operasi akan dikaji sebagai follow up sampai 1 tahun.

Metode

Obyek

Obyek primer penelitian ini adalah membandingakan irisan tranversal dan vertikal pada pembedahan abdominal dengan mempertimbangakan intensitas pengalaman nyeri abdominal yang pernah dialami pasien dan kebutuhan analgetik yang digunakan adalah kebiasaan prosedure operasi. Endpoint utamanya adalah intensitas pengalaman nyeri abdominal yang pernah dialami pasien, diukur dengan Visual Analog Scale (VAS) dan mendapatkan analgetik yang sama dalam (piritramide mg/H) pada hari kedua post operasi. Obyek sekundernya adalah frekuensi dari awal dan akhir dari onset komplikasi seperti peradangan pada perut, komplikasi post operasi pada paru, luka infeksi dan hernia insicional (Tabel 1).

Tambahan, nyeri diukur dengan skala sensasi nyeri Greissner, kuisiner nyeri yang dimodifikasi oleh Mc. Gill, design penelitian dilakukan di Jerman. Tempat

Penelitian dilaksanakan Department of General, Visceral, Trauma Surgery dan outpatient pada Universitas Heidelberg Medical School.

Partisipan

Total pasien adalah laki-laki dan perempuan berjumlah 172 orang, berumur lebih 18 tahun yang direncanakan untuk operasi abdomen elektif dan yang telah memenuhi syarat untuk dilakukan irisan tranversal atau vertikal.

Kriteria persyaratan

Kriteria Inklusi: Umur lebih dari 18 tahun, harapan hidup lebih dari 12 tahun, prosedur rencana yang akan diikuti pasien:

1. Whipple Procedure/Pylorus-preserve classic,

2. Reseksi duodenum-preserve pada kepala pankreas,

3. Partial/total gastrektomy

4. Reseksi kolon (kiri/kanan/tranversal)

5. Reseksi Ileocecal (laparatomi primer atau elektif, pasien harus diberi informed consent, pasien sudah di beri informed consent)

Kriteria Eksklusif:

1. Terapi permanen dengan obat opioid untuk beberapa alasan selama 12 bulan sebelum operasi (durasi lebih lama dari 2 minggu)

2. Incompatibiliti metamizole

3. Pembedahan abdomen berulang (Appendiktomi tanpa laparakospik, laparoscopic cholecystectomy, laparoscopic adrenalectomy, diagnostic laparoscopy atau appendiktomy), termasuk Scdan fannenstiel incici (histerektomy)

4. Partisipasi dari percobaan intervensi yabg lain yang akan mempengaruhi intervensi dan hasil dari studi ini

5. Beberapa penyakit psikiatri atau neurologi

6. Kurangnya gejala

7. Penyalahgunaan obat dan alkohol

8. Terapi immunosupresif berulang (> 40 mg kortikosteroid/hari atau azathioprine)

9. Kemoterapi dalam 2 minggu sebelum operasi

10. Radioterapi abdomen komplete > 8 minggu sebelum operasi (kecuali untuk terapi neoadjuvant, contoh: Ca pankreas)

11. Bedah liver, Gallbladder, Limpa dan rektum)

Etik, Informed Consent

Prosedur terakhir yang disetujui oleh Komite etik Independent Universitas Hedelberg Medical School . Informed consent akan diperoleh dari pasien baik secara lisan maqupun tertulis sebelum masuk dalam penelitian

Keamanan

Trauma abdomen, Infeksi paru, dan Infeksi luka adalah secondary end point, tetapi juga didefinisikan sebagai adverse events (kejadian-kejadian yang kurang baik). Trauma abdomen dan infeksi post operasi akan selalu dipertimbngakan sebagai adverse events.

Batasan adverse events meliputi beberapa tanda, gejala, dan sindrome, atau penyakit-penyakit yang muncul atau memburuk selama pasien berada dalam periode observasi di klinik penelitian dan semua itu mungkin merusak kesehatan pasien. Batasan adverse events juga meliputi temuan laborat atau hasil pemeriksaan prosedur diagnostik lainnya yang dipertimbangakan relevan.

Adverse events yang serius adalah adverse events yang terjadi pada waktu periode observasi yang menyebabkan kematian, ancaman kehidupan akut, menghendaki atau memperpanjang hospitalisasi, atau akibat-akibat pada ketidakmampuan yang sugnifikan dan ketidakcakapan.

Keamanan-data yang berhubungan akan dianalisa seseuai frekuensi pada:

1. Adverse events serius dan Adverse events yang distratifikasi menurut sistem organ

2. Adverse events yang distratifikasi menurut tingkat keparahan

3. Adverse events yang distratifikasi menurut penyebab

Analisa statistik

Hipotesis alternative untk primary endpoint adalah tidak ada perbedaan yang relevan pada nyeri postoperasi. Nyeri postoperasi mempunyai 2 komponen esensial: pengalaman nyeri pasien yang diukur pada hari ke-2 postoperasi dengan VAS dan sejumlah obat analgetik yang diprogramkan. Setelah itu, dua test equivalence harus dianalisa secara konfirmative. Pada kedua test tersebut pendekatan interval konfidence akan digunakan.Level signifikan harus 0,05. Prosedure Bonferrone-Holm akan digunakan untuk menganalisa masalah test multiple ini. Sejak randomisasi akan dikelompokkan oleh kelas-kelas operasi, analisanya juga harus dikelompokkan. Kemudian, sebuah analisa varian dengan 2 faktor tipe insisi dan kelas-kelas operasi akan ditampilkan.

Pergantian menjadi design superiority direncanakan jika salah satu test equivalence tersebut tidak memberi hasil signifikan. Analisa tersebut akan ditampilkan pada basic populasi (ITT) dan menyesuaikan dengan prinsip-prinsip ITT. Seorang pasien yang dimasukkan pada populasi ITT setelah insisi. Primary endpoint juga akan dianalisa pada basic sebuah populasi “perprotokol”. Metode deskriptive akan digunakan untuk mengkaji kualitas data, homogenitas tretment kelompok, endpoint dan keamanan pendekatan tranversal vs vertikal

Ukuran Sampel

Penghitungan sampel didasrkan pada a = 0,025 (Bonferroni-Holm) dan power yang dikehendaki 0,80. Penghitungan dilakukan secara terpisah pada masing-masing variabel primer menggunakan Query advisor version 4.0, Statistical Solutions Ltd., Cork, Ireland. Nilai maksimal dari penghitungan sample size seharusnya diambil sebagai sample size. Hasilnya adalah 86 pasien setiap group (lihat bawah).

Pengalaman nyeri

1. Design equivalence: Diasumsikan tidak ada perbedaan antara group means, SD 20 (VAS), dan equivalence margin d = 10 (VAS), 86 pasien setiap group dikehendaki.

2. Design superority: Diasumsikan sebuah perbedaan relevant secara klinik minimal adalah D = 10 (VAS) antara group means dan SD adalah 20 (VAS), 78 pasien setiap group yang dikehendaki.

Jumlah Piritrimide

1. Design equivalence: Diasumsikan tidak ada perbedaan antara group means, SD 1 (mg/H), dan sebuah equivalence margins d = 1 (mg/h), 23 pasien setiap group dikehendaki.

2. Design superiority: Diasumsikan minima perbedaan relevan secara klinik D = 1 (mg/h) antara group mean dan SD 1 (mg/h), 21 pasien setiap group yang diminta.

Untuk mencapai 86 pasien dalam 1 kelompok, sekitar 300 pasien terus dilakukan skreening berdasarkan data RS tahun 2000 kita melaksanakan randomisasi terhadap 15 pasien per bulan. Untuk mencapai jumlah sampel, pendaftaran akan berakhir ± 12 bulan, sampai Oktober 2004. Waktu berikutnya adalah 1 tahun. Pasien yang tidak diikutkan, tidak dirandom, atau tidak diikuti/follow up akan didokumentasikan dan dimintai keterangan khusus untuk penarikan kembali dari studi penelitian, sesuai pernyataan CONSURT.

Randomisasi

List randomisasi akan digeneralisasikan untuk semua strata dengan komputer (SAS Version 8.2, SAS Institute Inc, Cary, USA). List randomisassi yang sudah disegek (materai) akan diserahkan ke file penyelidik. Pasien yang dirandomisasi menggunakan amplop bersegel saat masuk ruang operasi setelah pembedahan abdomen disiapkan.

Blinding

Setelah pembedahan di ruang operasi seluruh abdomen akan ditutup dengan pembalut lua untuk menutupi luka insisi. Pembalut luka akan dibuka 1 kali post op yaitu setelah “primary endpoint” dikaji olh perawat blinded yaitu pada hari ke-2 post operasi. Jika perlu pembalut akan dibuka sebelum hari ke-2 postoperasi dan akan dilakukan oleh staff unblinded yang tidak terlibat selama parcobaan.

Program penatalaksanaan

Elektrocauterisasi akan digunakan untuk semua pasien sebagai standar incisi kulit. Pada pasien-pasien yang dirandom untuk mendekati median terpisah dari garis median. Pada kelompok incisi tranvese, otot rectum juga dincisi oleh cauterisasi. Waktu antara incisi pertama pada kulit dan saat penempatan penarik akan dicabut pada kedua kelompok.

Dinding abdomen akan ditutup dengan cara yang sama (standarisasi) pada kedua kelompok: Four Mikulic Clamps ditempatkan pada tepi facia abdomen dan kutaneus, teknik penutpannya dengan dua mono plus loups (BBD Aesculup, Tuttlingen, Germany), keduanya berakhir pada incisi dengan perbandingan/rasio 4:1 (suture, scar length), tidak ada penutup subkutaneus maupun drainase subkutaneus yang disertakan kulit akan ditutup dengan skin clips. Panjang skar diukur dalam cm

Pembalut luka ditempatkan sedemikian rupa sehingga menutup semua abdomen, termasuk drainase abdomen. Untuk perencanaan penanganan nyeri sudah ditentukan (distandarisasi) yaitu dengan metamizole (Novalgin, Aventis Pharma) 1 gr diberikan 4X sehari secara IV perinfus. Jika pasien mengalami reaksi yang tidak baik/abnormal, maka diberikan paracetamol (perfalgan, upsamedica EmbH) 1 gr per IV 4X sehari secara IV perinfus). Pasien tidak akan dikeluarkan dari trial jika obatnya diganti.

Monitoring

Pada studi independent, perawat tidak dilibatkan pada trial atau CRF (Case Report Form) secara lengkap akan memonitor pasien. Monitoring pembedahan dapat menjamin bahwa prosedur pembedahan yang sudah dibakukan sudah dilaksanakan secara benar, dan akan dilaksanakan oleh ahli bedah secara mandiri, dan tidak dilibatkan dalam trial

Follow up

Passien diobservasi selama 30 hari postoperasi, untuk menentukan onset komplikasi secara dini sebagai endpoint sekunder. Follow up ini akan lengkap pada 1 tahun setelah operasi dengan melaksanakan pemeriksaan fisik, termasuk USG untuk mengeksklusi incisi hernia.

Kesimpulan

POVATI TRIAL membandingkan 2 tipe incisi pada pembedahan umum. Perangkingan secara kualitatif dilaksanakan oleh pasien dan ahli bedah untuk mengkaji relevansi dari primary endpoint apakah dapat dimasukkan/tidak.

Rencana utama dalam design studi ini adalah:

1. Perspective pasien sebagai primary endpoint

2. Kalkulasi secara detail pada ukuran sampel yang adekuate

3. Randomisasi terstratifikasi untuk prosedur pembedahan terencana

4. Pengkajian pada pasien dan observer blinded pada primary endpoint

IMPLIKASI KEPERAWATAN :

1. Tugas perawat pada fase post-operative adalah merawat pasien dengan luka post operasi dalam hal ini adalah luka incisi di bagian abdomen. Perawatan luka secara tepat dimaksudkan untuk :

a. Melindungi luka dan trauma mekanik

b. Mengimobilisasi luka

c. Mengabsorbsi drainase

d. Mencegah kontaminasi dan kotoran-kotoran tubuh (feces, urine)

e. Membantu homeostasis

f. Menghambat atau membunuh mikroorganisme

g. Memberikan lingkungan fisiologis yang sesuai untuk penyembuhan luka

h. Memberikan rasa aman bagi mental dan fisik klien

Perawatan luka dan pengawasan terhadap kemungkinan terjadinya komplikasi pada kasus post operasi dengan incisi abdomen baik secara transversal maupun vertikal tidak dibedakan

2. Tugas perawat yang tidak kalah pentingnya dalam mengelola pasien post operasi adalah penanganan masalah nyeri post operasi. Ada beberapa teori manajemen nyeri yang dapat diterapkan oleh perawat, yaitu :

a. Non farmakologi

1) Menggunakan sistem pengkajian nyeri secara komprehensive : lokasi, karakteristik, onset/durasi, frekuensi, kualitas dan faktor-faktor presipitasi.

2) Mengobservasi tanda-tanda non verbal yang ditunjukkan klien yang berhubungan dengan ketidaknyamanan, khususnya apabila klien tidak mampu mengkomunikasikan ketidaknyamanannya

3) Menggunakan strategi komunikasi terapeutik untuk menggali pengalaman nyeri dan tunjukkan penerimaan terhadap respon nyeri klien

4) Deteksi adanya gangguan akibat pengalaman nyeri yang dialami klien, seperti : pola tidur, hubungan/komunikasi dengan oranglain, mood/perasaan.

5) Bantu klien dan keluarga untuk menyediakan sistem pendukung

6) Ciptakan lingkungan yang mendukung bagi klien untuk istirahat/tidur (lingkungan tenang, tidak banyak pengunjung)

7) Sediakan informasi yang jelas tentang nyeri, seperti penyebab nyeri, berapa lama nyeri akan berakhir, dan bagaimana mengantisipasi ketidaknyamanan akibat episode nyeri

8) Kontrol faktor-faktor lingkungan yang mungkin berpengaruh terhadap respon klien yang dapat menimbulkan nyeri/ketidaknyamanan

b. Farmakologi

Kolaborasikan dengan tim medis untuk pemberian obat-obat anti nyeri/analgetik sesuai kondisi klien, apabila diperluka

Screening versus routine practice in detection of atrial

fibrillation in patients aged 65 or over: cluster randomised

controlled trial

ABSTRACT

Objectives To assess whether screening improves the

detection of atrial fibrillation (cluster randomisation) and

to compare systematic and opportunistic screening.

Design Multicentred cluster randomised controlled trial,

with subsidiary trial embedded within the intervention

arm.

Setting 50 primary care centres in England, with further

individual randomisation of patients in the intervention

practices.

Participants 14 802 patients aged 65 or over in 25

intervention and 25 control practices.

Interventions Patients in intervention practices were

randomly allocated to systematic screening (invitation for

electrocardiography) or opportunistic screening (pulse

taking and invitation for electrocardiography if the pulse

was irregular). Screening took place over 12 months in

each practice from October 2001 to February 2003. No

active screening took place in control practices.

Main outcomemeasure Newly identified atrial fibrillation.

Results The detection rate of new cases of atrial

fibrillation was 1.63% a year in the intervention practices

and 1.04% in control practices (difference 0.59%, 95%

confidence interval 0.20% to 0.98%). Systematic and

opportunistic screening detected similar numbers of new

cases (1.62% v 1.64%, difference 0.02%, −0.5% to

0.5%).

Conclusion Active screening for atrial fibrillation detects

additional cases over current practice. The preferred

method of screening in patients aged 65 or over in primary

care is opportunistic pulse taking with follow-up

electrocardiography.

INTRODUCTION

Atrial fibrillation is a major risk factor for throm-

boembolic disease, particularly stroke.

Its prevalence

rises with age, from about 1% in the whole population

to about 5% in people aged over 65.

It can be

diagnosed by means of a simple low cost test

(electrocardiography), which is acceptable to most

patients. The risk of serious sequelae, such as stroke,

can be dramatically reduced by treatment, with a

68% relative risk reduction for ischaemic stroke and

25% reduction in relative mortality associated with

warfarin therapy compared with placebo.

Atrial

fibrillation therefore fulfils many of theWilson-Junger

criteria for a screening programme.

Two potential population screening strategies

include opportunistic case finding and systematic

screening. In opportunistic case finding, a healthcare

professional would take a patient’s pulse during a

consultation. If the pulse was irregular electrocardio-

graphy would be performed as a confirmatory test as

an irregular pulse is a non-specific predictor of atrial

fibrillation.

In systematic screening, the whole target

population would be invited for screening by electro-

cardiography.

One study in the United Kingdom compared sys-

tematic screening led by nurses with prompted oppor-

tunistic case finding for atrial fibrillation in primary

care (four practices, n=3001).The systematic screening

detected more cases than opportunistic case finding,

but atrial fibrillation had already been diagnosed in

many of the participants and the screening period

was only six months, making its generalisability

unclear.

Two further studies based in single practices

investigated the role of practice nurses in the screening

process7

and whole population screening8

: the first

study suggested that opportunistic screening with the

pulse taken by a nurse and 12 lead electrocardiography

was themost effective screening strategy,while the sec-

ond study investigated only population screening. It

therefore remains unclear which, if any, screening

strategy should be adopted.

As part of the screening for atrial fibrillation in the

elderly (SAFE) study, we tested whether screening was

more effective than routine care in detecting atrial

fibrillation in the community and, in a subsidiary

randomisation within the screening arm, compared

METHODS

Design and setting

This was amulticentred cluster randomised controlled

trial (figure) of computerised general practices in Eng-

land, with individual randomisation of patients in

intervention practices to two different screening strate-

gies.

Participants

We recruited 50 general practices from the Midlands

Research Practices Consortium (MidReC). All

patients aged 65 or over from these practices were eli-

gible for participation in the study, though patients

could be excluded if their own general practitioner

thought participation inadvisable.

Randomisation and sequence generation

Main trial

After stratification for practice size and deprivation

(based on Townsend score), we used MINITAB to

select randomly two equal size groups fromthose prac-

tices within a particular stratum.We used a simulated

value from a Bernoulli distribution, comprising two

values equally likely to occur, to determine which

group became the intervention arm (the other being

the control arm).

Subsidiary trial

After stratification for known atrial fibrillation (see

below) we randomly allocated patients from the 25

intervention practices to systematic or opportunistic

screening.

We obtained a computerised list of all patients aged

65 or over fromeach practice. Fromthese we intended

to identify random samples of 400 patients from each

intervention practice, and 200 from each control prac-

tice. As not all practices had as many as 400 eligible

patients we selected 440 patients from practices with

more than 900 eligible patients, 420 patients fromprac-

ticeswith between 420 and 900 eligible patients, and all

eligible patients frompractices with fewer than 420 eli-

gible patients. Random selection of patients for each

practice was achieved with computer generated ran-

dom numbers (MINITAB) to access entries in lists of

patients. For control practices only one practice did not

contribute 200 patients and no adjustment was made.

After initial sampling, we returned the lists of

patients to the practices and asked them to remove

the names of any patients who had died, moved, or

were terminally ill. These were replaced by a further

random selection of patients from a reserve list of 10%

drawn up for each practice. The cleaned lists fromeach

practice were classified into strata on the basis of

whether or not atrial fibrillation had been previously

diagnosed (see below). We used SPSS to allocate

patients randomly from this list to either systematic

or opportunistic screening to create two equal size

groups of patients within each stratum so that each

strategy (systematic or opportunistic screening) had

an equal chance of detecting known, unknown, and

suspected atrial fibrillation (n=4933). Which group

then became the systematic arm (the other being

opportunistic) was again decided by using a simulated

value from a Bernoulli distribution, comprising two

values equally likely to occur.

Before we flagged notes or invited patients for elec-

trocardiography, the general practitioners scrutinised

practice lists for a second time to remove any names of

patients who had died, moved, or were terminally ill,

and these patients were not replaced.

Blocking

We did not use explicit blocking in either themain trial

or the subsidiary trial but achieved balance at the selec-

tion stage by sampling equal numbers from stratified

lists of practices and patients.

Allocation concealment

There was no deliberate concealment of allocation to

the trial arms, but the trial management team did not

reveal the classification of atrial fibrillation to the par-

ticipants. The trial statistician determined allocation,

which was implemented by the trial coordinator

Identification of atrial fibrillation already known to

practices

We carried out computer searches of practice registers

to identify cases of atrial fibrillation in the 15 000 study

patients using a previously published strategy.

In

addition, we identified cohorts of patients receiving

prescriptions of digoxin, β blockers, class 1, 3, or 4

anti-arrhythmic drugs, aspirin, or warfarin. Clinical

records of patients thus identified as having atrial fibril-

lation or taking drugs that were associated with a diag-

nosis of atrial fibrillation in any of these computer

searches were reviewed for mention of a diagnosis of

atrial fibrillation. A diagnosis was accepted if there

were hospital letters referring to atrial fibrillation or

confirmatory electrocardiograms within the previous

five years.We reviewed an additional 5%randomsam-

ple of case notes of patients not identified on the com-

puter searches (750 in all) to estimate how many other

patients known to have atrial fibrillationwere not iden-

tified by the computer search strategy.

Intervention

Primary care physicians and othermembers of the pri-

mary healthcare team in the intervention practices

attended investigator days at which they were given

educational materials informing them of the impor-

tance of detecting atrial fibrillation and the available

treatment options. Members of primary healthcare

teams from control practices received no educational

input.

Practice nurses attended an electrocardiography

training day before they started screening clinics.

Training included how to perform electrocardiogra-

phy (with an electronicmachine to ensure standardised

high quality tracings) and basic interpretation of the

electrocardiogram (specifically how to identify atrial

fibrillation). Screening was undertaken in phases

because of the large number of practices, with the

first practice starting screening in October 2001 and

the last practice finishing screening in February 2003,

with each practice undertaking 12 months of screen-

ing.

All patients allocated to systematic screening

(including those with known atrial fibrillation) were

invited by post to attend a screening clinic. The notes

of patients in the opportunistic arm (including those

with known atrial fibrillation) were flagged with either

a manual paper flag or computer flag to encourage

pulse recording during routine consultation. Patients

with an irregular pulse were invited to attend a screen-

ing clinic. Once this process had been undertaken, the

flag was removed from the notes and returned to the

research team.

Practice nurses at the screening clinic obtained con-

sent from patients. Thus only those patients who

responded to the invitation in the systematic arm

(n=2357) and those with an irregular pulse in the

opportunistic arm(n=238) provided informed consent.

No data were available for those who failed to respond

and therefore did not give consent. All data relating to

patients were anonymised before they left the general

practice, and all research staff held honorary contracts

with the participating centres.

Practice nurses ran the screening clinics and col-

lected baseline information and medical history

(including any previous diagnosis of atrial fibrillation),

took the radial pulse, and carried out 12 lead electro-

cardiography (Biolog, Numed, Sheffield, UK). Two

consultant cardiologists (GL, MD), who were blinded

to allocation, assessed whether the electrocardiogram

showed atrial fibrillation or not and identified any

other relevant abnormalities. A third blinded cardio-

logist arbitrated on any disagreements over diagnosis.

Patients with normal findings on electrocardiography

were informed within two weeks and patients with any

abnormality were asked to make an appointment with

their general practitioner.

Main outcome measures

Our primary outcomemeasurewas the number of new

cases of atrial fibrillation detected during the 12month

study period (newly identified atrial fibrillation). To

detect new cases in control practices we carried out

computer searches of patients’ notes at baseline (see

above) and after 12months of screening.We reviewed

case notes of patients identified in these computer

searches for evidence of atrial fibrillation (using the

same criteria as above).

We assessed acceptability to patients using an

adapted version of the screening specific tool used in

the colorectal screening programme.

All patients

tionnaire immediately after screening.

Sample size

We assumed that the baseline prevalence of atrial

fibrillation known to the practice would be 3% (half

the expected prevalence of 6%11

) and that this preva-

lence would remain constant within the control

5%to detect a 1%difference in the detection rate of new

cases of atrial fibrillation between the different arms of

the study we required 5000 patients in 25 practices

(cluster size 200) in each group. This assumed an intra-

cluster correlation coefficient of 0.015 (derived from

prevalence data from a previous study).

Statistical methods

We analysed data on an intention to treat basis. We

subtracted any previously known cases of atrial fibril-

lation fromthe totals obtained at the end of the study to

prevent double counting in the newly identified atrial

fibrillation figures. Proportions and rates were used as

the measures of prevalence and newly identified atrial

fibrillation. We used χ2

test to compare overall

12 month prevalence and incidence rates between

arms.

Incidence analysis

We used non-linearmixedmodels with binomial error

to examine variation from arm to arm in the rates of

newly identified atrial fibrillationwithin the total popu-

lation minus the population with known atrial fibrilla-

tion (n=13 734), allowing for age (modelled as above or

below the median age (73.66) of those patients within

this reduced population) and sex and including both

main effects and interaction terms. Practice was

defined as a random effect to account for extra bino-

mial variability at the practice level. Any difference in

baseline prevalence at practice level was accounted for

with the inclusion of the log odds of baseline atrial

fibrillation. These analyses were conducted using

Proc Nlmixed in SAS (version 8.2) (SAS, Carey, NC).

The analysis was repeated without the inclusion of the

covariate describing baseline prevalence.

We used logistic regression to compare rates of

newly identified atrial fibrillation per practice in the

control and intervention arms and also in a matched

pairs analysis of 12 month incidence between the

opportunistic and systematic arms.

RESULTS

Participant flow

Table 1 gives details of age and sex of the study parti-

cipants by screening arm. We reviewed case notes of

5216 (35%) of the total study population at baseline

because they had been picked up on computer search

as having atrial fibrillation or relevant drug code.

Baseline prevalence of atrial fibrillation

We confirmed atrial fibrillation in 1068 patients

(20.5% of search positive population, and 7.3% of

total population) by reviewing clinical records. Case

notes were unavailable for 21 patients. In the addi-

tional random 750 sets of case notes that we reviewed

as a validation of the computer searches, we found no

extra cases of atrial fibrillation. The baseline preva-

lence was 7.9% (95% confidence interval 7.2% to

8.7%) in the control arm and 6.9% (6.2 to 7.6%) in

both the opportunistic and systematic arms. The intra-

class correlation was 0.0027 across all practices.

Newly identified atrial fibrillation over 12 months

Table 2 shows the number of new cases of atrial fibril-

lation found through searching notes at 12 months

after the baseline search.We calculated rates of newly

identified atrial fibrillation after removing those

patients in whom atrial fibrillation was diagnosed at

baseline and patients whose notes could not be

found. Newly identified atrial fibrillation therefore

refers to the number of new cases diagnosed within the

12month study period. Data weremissing for patients

whose notes were no longer available because of death

or no longer being at the practice. The overall mean

12 month prevalence was 8.9% (7.9% to 9.7%) in the

control arm, 8.4% (7.6% to 9.4%) in the opportunistic

arm, and 8.4% (7.6% to 9.3%) in the systematic arm.

The detection rate of new cases of atrial fibrillation

was 1.63% a year in the intervention practices and

1.04% in control practices (difference 0.59%, 0.20%

to 0.98). Systematic and opportunistic screening

detected similar new numbers of cases (1.62% and

1.64%; difference 0.02%, −0.5% to 0.5%) (table 2).

After we accounted for possible practice to practice

variation using amixedmodelwith binomial error var-

iation,we found a significant difference in the detection

of new cases of atrial fibrillation between the control

arm and the opportunistic and systematic arms

(P=0.01 and P=0.02, respectively) but not between

the opportunistic and systematic arms (P=0.95).

Within each of the three arms there was no significant

practice to practice variation (P=0.88, P=0.62, and

P=0.96, respectively). After we accounted for baseline

prevalence, screening practices identified substantially

more cases of atrial fibrillation than the control prac-

tices (odds ratio 1.61, 1.14 to 2.29, P=0.0085), and we

obtained similar results when we removed baseline

prevalence from the model (1.58, 1.12 to 2.22,

between patients from the same practices allocated to

either the opportunistic or systematic arms showed no

significant difference in detection of new cases of atrial

fibrillation at 12months (P=0.67) or between practices

(P=0.60).

Opportunistic v systematic screening

We identified 75 new cases of atrial fibrillation in the

opportunistic screening group and 74 new cases in the

systematic screening group. Differences in screening

method had no effect on the rate of detection between

the groups (odds ratio 0.99, 0.72 to 1.37, P=0.95).

In total 3278 (69%) patients were opportunistically

screened for atrial fibrillation during the 12 months of

the study. After they had had their pulse taken, 122/

360 (34%) patients with an irregular pulse declined

electrocardiography. Of the remaining patients, atrial

fibrillation was diagnosed in 84/238 (35%) on electro-

cardiography (2.6% of patients who had their pulse

taken). Of patients who had an irregular pulse but no

electrocardiography, atrial fibrillation had already

been confirmed in 56/122 (46%). Of the 75 new cases

of atrial fibrillation in the opportunistic arm, 24 had a

regular pulse recorded but subsequently had atrial

fibrillation diagnosed outside the screening pro-

gramme. The irregular pulse could have been missed

in these 24 patients; or they could have developed

atrial fibrillation subsequent after they had their pulse

taken; or theymay have had paroxysmal atrial fibrilla-

tion.We cannot distinguish between these three possi-

bilities.

Of the 4433 patients invited for systematic screen-

ing, 2357 (53%) underwent electrocardiography, 904

(20%) declined screening, and 992 (22%) did not

respond. Some 135 (6%) were found to have atrial

fibrillation, which was a new diagnosis in 52 (39%)

(the 22 additional cases were detected outside the

screening programme).

Prevalence of paroxysmal atrial fibrillation

Of the 339 intervention patients confirmed to have

atrial fibrillation at baseline and randomised to

systematic screening, 205 underwent screening elec-

trocardiography during the study, which confirmed

atrial fibrillation in 136 (66%). The electrocardiogram

showed sinus rhythm in 69 (34%). Thus, about a third

of prevalent atrial fibrillation at baseline was either self

limiting or paroxysmal.

All patients undergoing electrocardiography

(n=2595) were given a questionnaire after screening,

of which 1940 (75%) were returned completed. Most

(95%) respondents thought that screening was impor-

tant and that sufficient information was available

before screening.

DISCUSSION

In this multicentred study we have identified the most

effective manner of screening for atrial fibrillation in

patients aged 65 or over. Opportunistic screening

with opportunistic pulse taking, prompted by flagged

case notes, during routine clinic attendances and then

performing 12 lead electrocardiography on those with

an irregular pulse was as effective as systematic popu-

lation screening, with both being more effective than

“routine practice,” which comprised non-systematic

case finding. This finding suggests that routine electro-

cardiography within this population is unnecessary for

the detection of atrial fibrillation as long as healthcare

professionals are conscientious about feeling the pulse.

The utility of opportunistic screening was undoubt-

edly due in part to the extensive coverage attained over

the 12month study period (69%) in contrast to a lower

coverage rate (29%) over a six month period in the

Southampton study.

Within SAFE opportunistic

pulse taking was encouraged by the flagging of clinical

notes. This was predominantly manual, though with

the increase in the use of wholly computerised records,

and improvements in practice administration driven

by incentives, such as the quality and outcomes frame-

work in the UK, the potential for opportunistic screen-

ing will probably increase. The validity of our findings

may be transferable to any system of managed care

where high rates of coverage can be achieved.

Therewas no significant difference in themean com-

munity prevalence of atrial fibrillation at 12 months

valence was 6.9%-7.9% in those aged 65 or over. The

SAFE prevalence rates of atrial fibrillation represent

more robust estimates than the previously reported

prevalence in this population of 4.5% from systematic

screening6

and 5.4% from total population screening.

Routinely reported data for those aged 85 or over has

suggested a prevalence of atrial fibrillation of around

10%,

significantly lower than we found in our study.

These earlier studieswere small and in less investigated

populations and probably systematically under-

reported atrial fibrillation, whereas the SAFE popula-

tion was, by design,much larger andmore representa-

tive than earlier studies with more robust methods of

screening. General practitioners removed more

patients from the opportunistic screening arm than

from the systematic screening arm, although the same

criteriawere used for removal.This discrepancy is rela-

tively small and does not affect our main outcome.

Little information is available on the rate of newly

identified atrial fibrillation. One small Scottish study

in younger people (45-64) reported a four year

incidence of 0.54 per 1000 patient years.

In this

study there were also significant differences between

the populations, with the detection rate being

significantly higher in screened populations than in

routine practice.

The prevalence and incidence rates for atrial fibrilla-

tion in SAFE are therefore likely to represent reliable

estimates for theUK(table 3).The screening processes,

whether systematic or opportunistic, did not raise anxi-

ety and were acceptable to patients. As the detection

rates were essentially identical for the two methods,

however, the more labour intensive, costly, and intru-

sive approach with systematic screening cannot be jus-

tified. Furthermore, systematic screening was much

less acceptable to patients overall, with over 20% posi-

tively declining the offer and over 26%not responding

to the offer.

Limitations of study

Our study was complicated by the fact that the control

population had a higher prevalence of atrial fibrillation

at baseline, which we assumed to be a randomfinding.

This had to be corrected for in the analysis. Further

study limitations included the inability to detect all

cases of paroxysmal atrial fibrillation (although this

was presumed in patients whose case notes indicated

atrial fibrillation but whose electrocardiograms were

negative) and the poor uptake of systematic screening.

These data do not directly tell us how often screening

should take place, though this will be estimated in

future economic modelling of the clinical yields on

the basis of the observed new detection rates.We will

also model the effect of antithrombotic treatment on

the incident cases. This will affect the cost effectiveness

of any screening programme, given that the earlier

atrial fibrillation is detected the sooner antithrombotic

treatment can start. It is not clear how uptake in

patients would be affected if this became a routine pro-

cedure. Patientsmay have been reluctant to participate

in a research project as opposed to undertaking a rou-

tine screen. Uptake of cervical screening, for example,

improved dramatically over the past decade as it

became routine.

ABSTRAK

Tujuan Untuk menilai apakah penapisan meningkatkan Deteksi fibrilasi atrium (randomisation cluster) dan untuk membandingkan skrining sistematis dan oportunistik. Desain cluster Multicentred acak terkendali sidang, dengan percobaan anak tertanam dalam intervensi
lengan.

Menetapkan 50 pusat perawatan primer di Inggris, dengan lebih lanjut randomisation individu pasien dalam intervensi praktek. Peserta 14 802 pasien berusia 65 atau lebih dalam 25
praktek intervensi dan 25 kontrol. Intervensi Pasien dalam praktek-praktek intervensi yang

PENDAHULUAN

METODE
